HMSA’s credentialed provider network consists of over 14,000 professionals throughout the United States and Puerto Rico to support our staff and ensure that we meet the needs of our clients. As a national provider to hundreds of diverse organizations, our clinical network must meet strict credentialing processes to ensure we deliver 100% satisfaction to our clients. The requirements needed to join our network would apply: - Minimum of a Master's level degree in a behavioral health related field
- At least three years post-graduate experience
- Current state license/certification at the highest level in your state
- Malpractice insurance coverage of at least $1 million per incident, $1 million aggregate

4. Agreement and Certification of Equal Employment Opportunity (PDF)

6. Completed W9 (Must be returned for payment processing)
